Mental Health America of California (MHAC) and the California Youth Empowerment Network (CAYEN) is partnering with the National Resource Center on ADHD (NRC) (a program of Children and Adults with Attention-Deficit/Hyperactivity Disorder (CHADD)), a national clearinghouse for the latest evidence-based information on ADHD. The NRC is seeking feedback from youth, who have been at one time diagnosed with ADD or ADHD. Your feedback will help to inform national efforts to create services, supports and programs designed to support people with ADHD and their families.

Participants will receive a link to a survey to complete by September 8th. You will not be required to submit personal information for the survey; however, you will be asked for basic demographic information and will be asked for your name and address so that we can send you your $50 stipend check. Your name and address will not be provided to NRC.
